how to make a claim

This section is for employees who have a one fund and would like to make a claim.

If you need to make a claim on your one fund plan for dentists’ fees, opticians’ fees or any other treatments covered by your plan, you can rely on us to act quickly. Our friendly, polite team are always on hand to help and there are no complicated forms to wrestle with.   

Claims are submitted using our simple claim form. You can download a claim form here or call us on 0800 988 2128†.

All claims must be submitted within 3 months of the treatment date on the original receipt - amended or photocopied receipts will not be accepted. The original receipt must show your full name and address details. Photocopies, faxes, credit card vouchers and till receipts are not accepted.
