how to make a claim with our health cash plan

If you need to make a claim on your health cash plan for dentists’ fees, opticians’ fees or any other treatments covered by your plan, you can rely on us to act quickly. Our friendly, polite team are always on hand to help and there are no complicated forms to wrestle with.

Claims are submitted using one of our simple claims forms. You can download a claim form below, from useful documents or call us on 0800 988 2129†.

All claims, with the exception of personal accident cover and life cover, must be submitted within 13 weeks of the date on the receipt. Your original receipt must show your full name and address details. Photocopies, faxes, credit card vouchers and till receipts are not accepted.

Claims for personal accident cover must be submitted within 3 months of the incident.

Claims for life cover must be supported with an original or certified copy of the death certificate.
